BUTUAN CITY – 10/29/2020

A week after declaring office lockdown on September 1-4, 2020, DepEd Caraga Regional Director Francis Cesar B. Bringas, CESO V directed all regional office personnel as well as Agusan del Norte Division personnel who are temporarily housed at the regional office in Butuan City to accomplish a Weekly Health Declaration Checklist every Monday starting September 7, 2020.  

The said office lockdown aimed to disinfect the entire regional office complex, done in coordination with the City Government of Butuan City. On the resumption of office operation on September 7, 2020, per Office Memorandum no. 09-055, s. 2020, RD Bringas reiterated his directive to every employee to strictly adhere to the standard health protocols such as wearing a face mask or face shield, observing social distancing, frequent hand-washing, and no unnecessary office-hopping.

The checklist includes questions if employees had travel outside and/or anywhere in the country for the last 14 days; if he/she has possible close contact or had been exposed to any person who traveled outside Butuan City, among others. Employees were also required to declare if they have comorbidities and if they experienced symptoms such as colds, fever, sore throat, and other COVID-19 known symptoms.

The Education Support Services Division (ESSD) monitors the weekly Health Declaration to ensure strict compliance.
